Notice Regarding Availability of IRS Forms 1095-B and 1095-C
In accordance with Internal Revenue Service (IRS) guidelines, Northwest Construction provides Forms 1095‑B and 1095‑C upon request.
Employees and other individuals who are entitled to receive a Form 1095‑B and/or Form 1095‑C may request a copy at no cost by contacting the Human Resources Department using one of the methods below:

Upon receipt of a request, Northwest Construction will furnish the requested form within 30 days, or by the applicable IRS deadline, as required by law.
These forms are used for informational purposes related to health coverage and are not required to be filed with your individual income tax return. However, you should retain them with your tax records.
